In the event of disqualification from driving please contact AXA Assistance Claims
Centre Limited within 31 days of disqualification to notify loss of licence.
Please fully complete, sign and date the claim form and return it with all supporting
documentation - outlined below to: -

Supporting documentation: -
- Copy of your Certificate of Insurance.
- Copy of the Certificate of Motor Insurance issued by your Car Insurer with proof
you have held a private motor policy for a minimum of two (2) years.
- Copy of your driving licence valid in the United Kingdom.
- Copy of your passport
- Evidence of the period of disqualification
Documentation as evidence of alternative transport costs: -
- You must submit a daily account listing the journeys made and the cost
- Where you use a nominated person to act as a driver you must produce documentation
to confirm that the driver has been employed by you, that the monies have passed
from you to the driver, and that they are complying with all National Insurance
and taxation regulations.
- Where you use methods of transportation other than a nominated person to act as
a driver you may be requested to provide evidence that the transport you have chosen
complies with all licensing and insurance regulations.
Failure to follow these steps may jeopardise the reimbursement of your costs.
